Liability Waiver and Insurance Acknowledgment I understand and agree that Wee Dance Academy is not responsible for any injuries, accidents, or medical conditions that may occur during classes, rehearsals, or performances, whether on or off the premises. I acknowledge that it is my responsibility to obtain appropriate health and accident insurance for myself or my child. By submitting this form, I hereby release, discharge, and hold harmless Wee Dance Academy, its instructors, staff, and affiliates from any and all claims, damages, or liabilities arising from participation in any class or event associated with the Academy. I have read and agree to the liability disclaimer. I understand that Wee Dance Academy is not responsible for any injury or medical issue during class. I confirm the student is fit to participate and I will provide my own insurance if needed.
